Provide Case management activities with geriatric clients through home visits and telephone calls.Ensure that clients receive the range of appropriate services to meet their needs by maintaining good working relationships with clients, medical staff, social service agencies, and community agencies.Providing quality of services for clients through the development and review of care plans.Identifies client needs and problems through supportive face to face or telephone contact and provides alternative ways of resolving problems, including intervention, negotiation, and advocacy with providers on the client’s behalf to ensure the delivery of needed services and benefits along with emotional support.Provides information about services, benefits, entitlements, and other areas of concern to older person or their representatives to enable them to locate and obtain needed resources on their own and/or with assistance.Participates in community outreach representing Agency and Department, creating ongoing partnerships with community professionals.Assist with training for newer and existing employees as needed. Detail orientated documentation and communication when discussing with team leaders.
